Overview

Stage A: Review of Five Patient Charts

In STAGE A the physician compares his or her current practice (through a review of chart documentation) to guidelines and measures provided for the topic area. No patient/chart data is submitted. When completed, STAGE A is recorded in the individual’s CME transcript and CME credit is awarded.

 

Instructions:STAGE A is provided in the workbook. Enter information about your charts on the form in the workbook.Choose five adult patients as indicated. Use STAGE A to review the charts for these five adult patients to determine if care was consistent with evidence-based recommendations.

 

Scoring: In the TOTAL column, tally the total number of check marks in each row. For any row for which the total is less than 5, examine whether clinical or other circumstances explain why practice in this area was not consistent with recommended care. If not, consider whether changes or use of any of the suggested clinical tools in your practice could strengthen the provision of evidence-based care.After the chart review step is complete, continue to the evaluation, document your participation, and receive a STAGE A certificate of credit. Proceed to STAGE B, Improvement Plan.

To earn credit, complete the Chart Review and Stage A Evaluation. Proceed to Stage B - Improvement Plan.
